2022 Registration

DIETARY CONCERNS
Make note of any dietary concerns below. Please note that Aramark and the UMass Lowell dining services make reasonable efforts to accommodate dietary concerns or issues. We cannot accommodate any dietary concerns received after June 12, 2022.

JAZZ ENSEMBLES
Students may audition for a jazz ensemble. Students who do not audition or do not earn a spot in a jazz ensemble will be placed in an ensemble specific to their instrument. These ensembles will take place at the same time period.

Participation Agreement
Participation in this camp is a commitment to the ensembles and activities. I (my child) understand that I am expected to be fully present for the duration of the camp.

Photo/Media Release
I hereby grant the UMass Lowell Symphonic Band Camp, irrevocable permission to record and use my child’s name and photograph, in all forms and media, including but not limited to publications, brochures, websites, magazines, exhibitions, videos, digital media, social media such as Facebook, Twitter and Instagram, or any other media, for the purpose of education, marketing, advertising, communication, public relations, publicity, training or any other lawful purpose. I hereby waive any right to payment, compensation or royalties arising out of or related to the use of my child’s likeness by the UMass Lowell Symphonic Band Camp.

I agree to comply with the behavioral standards of UMass Lowell / UMass Lowell Symphonic Band Camp, and respect
the underlying assumptions and principals upon which they are based. The Camp Director & Assistant Directors reserve the right
to dismiss a student from the program, without the possibility of a refund, for failure to comply with rules and guidelines provided
to them by the camp staff throughout the week. Parents will be contacted and are expected to pick up their child at any time in
which it is deemed necessary for the student to be removed.
